Release of Liability

A release of liability (waiver) is designed to protect the university from legal liability and to inform participants of risks associated with certain activities. To facilitate a consistent application of risk control among CSU campuses, the Release of Liability, Promise Not to Sue, Assumption of Risk and Agreement to Pay Claims was developed.  

A waiver must be signed by student participants in university planned or sponsored off campus activities, i.e., academic field trips, regardless of the activity involved.

Signed waivers must be kept on file in the department sponsoring the activity for three years following the activity (if the participant is a minor the waiver must be maintained for three years or until age 20, whichever is longer).    

Release of Liability – COVID-19 OnSite Clinical Student

The COVID-19 OnSite Clinical Student release of liability (waiver) is designed to protect the university from legal liability and to inform participants of risks associated with voluntary participation in OnSite Clinical assignments during the COVID-19 pandemic. 

A signed waiver is required prior to any participation in an OnSite Clinical assignment.

Signed waivers must be kept on file in the department sponsoring the activity for three years following the activity (if the participant is a minor the waiver must be maintained for three years or until age 20, whichever is longer).
