Release of Liability (Waiver)
A Release of Liability (Waiver) is designed to protect the university from legal liability and to inform participants of risks associated with certain activities. To facilitate a consistent applicaton of risk control among CSU campuses, the Release of Liability, Promise Not to Sue, Assumption of Risk and Agreement to Pay Claims was developed.
A waiver may be required for on campus activities, as determined by a Risk Assessment. Please submit completed Risk Assessment form to Nancy Demich by E-mail at firstname.lastname@example.org or fax at 619-594-6022. Examples of activities that will normally require a waiver include:
- events that include physical activity
- events that include rides or inflatables
- events that include colored cornstarch
- events that include minors
Signed waivers must be kept on file in the department sponsoring the activity for three years following the activity (if the participant is a minor the waiver must be maintained for three years or until age 20, whichever is longer).
