Accommodation and Arrangement Requests
Request for Accommodation:
Please fill out the Reasonable Accommodations form which includes physician certification for accommodation for your own disability and email the completed form to [email protected].
Request for an Arrangement:
Please fill out the Arrangement google form if you are in need of an arrangement due to dependent care or have other reasons for request (child/dependent care, immunocompromised family, etc.) Once we receive a completed submission, an OERC team member will follow up with you shortly.
