Authorized Medical Providers

For Work-Related Injuries

Injuries Requiring Emergency Treatment

Most injuries do not require emergency treatment. In those non-life threatening situations, the employee must be referred to one of the Sharp Rees-Stealy, Occupational Health Services Facilities listed below, unless the employee has pre-designated their primary physician, in writing, prior to the industrial injury.

Control of Medical Treatment

The California Labor Code states that the employer has control over the employee’s medical treatment, for the first 30 days following an injury. After 30 days from the date the injury was reported, an injured employee may be treated by a physician of their choice. The employee must submit a written request to the university claims adjuster:

An employee may designate initial medical treatment with their own physician, provided they have submitted a Notice of Physician’s Pre-designation Form Active link to a Portable Document Format file with a .pdf filename extension created using an Adovbe Acrobat product prior to the injury. Forms must be on file with the Center for Human Resources and with the employee’s supervisor.

If an employee goes to a physician not affiliated with SDSU Health Services, Sharp Rees-Stealy Occupational Medical or Alvarado Hospital, the university cannot guarantee that their medical bills are covered under workers’ compensation.
