A variety of Health Information is available to Anthem Blue Cross members (employees enrolled in PERS Select, Choice or Care health benefits) including condition management programs, healthy living resources and special offers.

Health Assessment

Anthem Blue Cross members have 360° Health as your health assessment option. PERS Select members receive $100 debit card for completing the assessment and any CalPERS member of Anthem Blue Cross will be entered into a quarterly drawing for a $500 debit card.

Smoking Cessation

Anthem Blue Cross has developed a tobacco cessation program, TLC: The Last Cigarette Program to support employees with smoking cessation efforts. For more information call 866-634-3435. TLC 4 YA is on online resource designed specifically for teens and young adults.
