Employee gains or loses alternative non-CSU coverage.
A change in work site or residence resulting in eligibility or cessation of the employee's coverage under any health maintenance organization offered through the plan.
Employee is deleted as a dependent from a CalPERS health plan.
Spouse, domestic partner, or dependent loses or obtains non-CalPERS health coverage.
