Authorized Medical Providers for Work Related Injuries

Most injuries do not require emergency treatment. In those non-life threatening situations, the employee must be referred to one of the Sharp Rees-Stealy Occupational Health Services Facilities or WorkMed Industrial Care Providers listed below, unless the employee has pre-designated his primary treating physician, in writing, prior to the industrial injury.

All life threatening injuries should immediately be referred to Alvarado Hospital Emergency Room @ 6655 Alvarado Road, San Diego, California, 2293130. Call 911 if calling from campus, followed by a call to EH&S at 619-594-6778.

Labor Code states that the employer has control of medical treatment for the first 30 days following an injury. After 30 days from the date the injury was reported, an injured employee may be treated by a physician of his/her choice. The employee must submit a written request to the claims adjuster at Octagon Risk Services, Inc., P.O. Box 15368, Long Beach, CA 90815.

An employee may designate initial medical treatment by their own physician if the employee has submitted a “Notice of Physician’s Pre-Designation” form prior to the injury. Forms are available in the Center for Human Resources and on our website at http://bfa.sdsu.edu/ps/wc/PhysicianPreDesignationNotice.pdf, and must be on file in both the Center for Human Resources and the immediate supervisor’s office.

If an employee goes to a physician other than an SDSU Health Services, Sharp Rees-Stealy Occupational Medical or WorkMed physician, or facility other than Alvarado Hospital Emergency Room in the case of life threatening injuries, we cannot guarantee the medical bill(s) will be covered under workers’ compensation.
