The California State University Dependent Care Reimbursement Account (DCRA) and Health Care Reimbursement Account (HCRA) allow employees to pay certain dependent and health care expenses from their salary on a pre-tax basis. HCRA and DCRA are also referred to as Flexible Spending Accounts (FSAs).
Employees may contribute any amount from a minimum of $20 to a maximum of $416.66 per month ($5,000/year) for each account. A monthly administration fee of $1.00 will be deducted from your salary on an after-tax basis. Employees must re-enroll each year to establish their HCRA/DCRA accounts.
To participate, complete the Dependent Care/Health Care Reimbursement Account Plans Enrollment Authorization Form
and turn in the original to the Benefits office located in Extended Studies Center, 4th floor or send via campus mail to MC 1625. Due to legal requirements, we are unable to accept a faxed form. Please note: Benefits staff will complete the “Deduction/Organization Code” information on the form.
Application Software, Inc. (ASI) is the Third Party Administrator for the HCRA/DCRA benefit. A Dependent/Health Care Reimbursement Account Claim Form
can be faxed or mailed to ASI. You may choose to receive your reimbursement via check or direct deposit. For direct deposits, employees will need to complete the Direct Deposit Form for Dependent Care/Health Care
