Mandatory Dependent Social Security Number
The Chancellor’s Office has notified us in Technical Letter HR 2011-05 
that campuses are required to collect the Social Security Number of any individual(s) enrolled in a CalPERS health plan, CSU dental and/or vision plan as the dependent of a benefits eligible employee. This policy is applicable to new hires and existing employees. If an employee elects coverage for his/her dependent(s) during open enrollment and does not provide valid Social Security Number(s) for the dependent(s), the campus has been instructed to not process the open enrollment request until proper documentation is provided.
