Effective January 1, 2011, health, dental and vision benefits were extended to eligible dependents up to age 26. You have an opportunity to add an eligible dependent(s) to your health, dental and/or vision enrollment during the Open Enrollment period which is held from October 10, 2011 through November 4, 2011. Benefits for newly added dependents will take effect January 1, 2012.

Employees who wish to add a dependent during this year’s open enrollment period must provide the dependent’s social security number and a copy of the dependent’s birth certificate to the Benefits Office.
