Control of Hazardous Energy Sources (Lockout/Tagout Program)
VI. RESPONSIBILITIES
A. Environmental Health and Safety (EH&S) is responsible for:
- Preparing and maintaining this written program which complies with the requirements of applicable Cal/OSHA requirements.
- Working with directors, managers, and supervisors to ensure employees are trained on the requirements of this program.
- Assisting with annual reviews of this written program.
B. Directors are responsible for ensuring that the written standard operating procedure in this document is followed.
C. Managers and supervisors are responsible for:
- Designating personnel as “authorized employees” and identifying those personnel who are considered “affected employees.”
- Ensuring that all authorized and affected employees have received proper training on the Lockout/Tagout Program.
- Identifying and maintaining an inventory of all machinery or equipment subject to lockout/tagout requirements. This includes the location, description, power source, primary hazards of the equipment, a list of the primary operators and maintenance personnel, and a list of lockout/tagout equipment that is used and maintained on site.
- Ensuring that all applicable new or overhauled equipment is capable of accommodating lockout devices.
- Ensuring that all authorized employees are issued appropriate locks and tags.
- Providing and maintaining necessary equipment and resources, including locks, tags, and blocks.
- Reviewing the Lockout/Tagout program and energy control procedures annually to ensure that the procedure and requirements of this program are being followed.
- Ensuring that lockout tagout protection is maintained during shift or personnel changes.
D. Authorized and affected employees are responsible for:
- Complying with the provisions of this program.
- Reporting to their supervisors any equipment, conditions, or energy sources where lockout/tagout is not being used as required.
- Maintaining lockout/tagout supplies in maintenance vehicles.
E. Contract and Procurement Management is responsible for ensuring that all bid specifications and or the resulting agreement will contain language requiring the contractor to adhere to Cal/OSHA requirements.
F. Any outside contractor and or servicing personnel performing work covered by the scope and application of this policy on university property shall adhere to the applicable requirements of Title 8 of the California Code of Regulations and follow campus procedures.
- Contracts Not Competitively Bid: All contractors shall provide copies of their IIPP and any associated written safety programs specific to the work to be performed to the requesting department with their proposal/quote for services. Contract and Procurement Management will make the documents available to Environmental Health and Safety.
- Contracts Competitively Bid: When a contract is awarded as a result of competitive bids, contractors shall provide copies of their IIPP and any associated written safety programs specific to the work to be performed to Contract and Procurement Management prior to award of the contract. Contract and Procurement Management will make the documents available to Environmental Health and Safety.
